FOR PATIENTS –
Lockridge Medical Centre has commenced phase 1b of the COVID vaccination program.
In the early stages of Phase 1b the supply of COVID vaccines to general practices will be in limited supply.
To receive the vaccine patients will be required to show proof of eligibility. If you don’t normally attend our practice we will require you to bring a letter or health summary from your usual GP. If you are eligible based on your profession you will need to provide proof of employment.
The following patients will be eligible in this phase:
- People 70 years or older
- Health care workers
- ATSI over 55 years
- Younger people with an underlying medical condition or disability
- Critical and high-risk workers

We will only be able to vaccinate you once you are certain that you want to have it. ONLY book once you have read and agree with the consent form below.
